2010 Success Award Nomination Form

  • Recognize the achievement of your students at the 2010 Student Success Event on Wednesday, June 2, 2010, 6:30p.m. at the Cypress College Theater.
  • Honor students who excel academically and/or who have overcome significant barriers in order to continue their education.
  • Instructors and staff members may nominate more than one student, but a separate nomination form must be filled out for each individual student that they nominate.
  • All students nominated and NOT selected will receive a recognition letter from the Provost.

Please provide a brief paragraph that includes the essence of your student and their outstanding achievement/success. When writing your paragraph, remember, the more you include about the student, the better chance the committee has to evaluate their nomination.
Please include the following criteria when writing your student nominations:

  • Overcome life challenges or persistence in spite of obstacles (e.g. medical/financial concerns, distance traveled, family obligations, etc.)
  • Positive role-model and/or inspires others to meet their potential (e.g., leadership skills, etc.)
  • Skills attained; academic rigor, class performance and application of theories learned in class.
  • Excels beyond expectations or class requirements.

A sample of a selected student nomination paragraph has been provided below:
Judith Gonzalez, a mother of two, postponed her own educational dreams for 17 years to support her family by working in a local plastic factory. Three years ago, Judith had the opportunity to enroll in English-as-a-Second Language (ESL) classes at SCE. She attends class everyday and often models assignments such as role plays for other students. She is respectful towards class members and often leads discussions and class projects without monopolizing them. Her overall test scores are among the top in the classroom. Judith has also done volunteer work as a translator at a local high school, taking every opportunity to practice her new English skills.
Cathee Mang, Judith’s ESL teacher, relays that she is “impressed with her ability to incorporate new vocabulary and grammar into her speaking and writing, both of which are excellent.” Judith plans to earn her certificate in Early Childhood Education to be a preschool teacher and then move onto college to earn her teaching degree.
